Job Summary

The Hartford School District is seeking a licensed Social Worker to support students and families through comprehensive case management coordination of community services and collaboration with school teams. This role focuses on addressing non-academic barriers that impact student access to education attendance wellness and management for students with complex needs involving multiple agenciesKey Responsibility Include:- Coordination of medical mental health and community-based supports- Facilitation of Coordinated Services Plans (Act 264) and attendance team meetings- Collaboration with principals counselors special educators and district leaders- Home-based wellness checks and support with truancy-related concerns- Advocacy for students and families including marginalized populations- Liaison with hospitals treatment facilities and service providers- Staff consultation and professional development- Identification and reporting of child abuse and neglectThe Hartford School Districts Anti-Racism Policy believes that the educational experience of every student is enhanced through the professional contributions of representatives of various races ethnicities genders physical abilities ages and sexual orientations and that a diverse and culturally competent faculty is essential in the creation of a rigorous learning environment free of bias and inequities.
