HIM Clerk
Position Summary
The Health Information Management Clerk position is responsible for any combination of the following activities: Assist and support HIM Techs with discharge processing prepping scanning and filing. Prepping scanning and indexing of loose documents that are sent after discharge to the HIM department. Good understanding of paper documents and how they are used within EHR. This position will also assist with department projects as needed and with supervision.
Major Responsibilities
Assist in Prepping Scanning Index and Analysis 60%
- Answer phones
- Customer service assistance with visitors to the department to include physicians and leadership
- Understanding of PHI and how it applies to release of patient information
- Demonstrates the ability to send fax requests ROI through MPF/ROI Client.
- Prepping of loose documents includes sorting by document type removing staples etc.
- Analyzing OPTS records within MPF
- Retrieval and processing of discharged patient as needed
- Good understanding of paper vs electronic records
- Assist with pulling and filing paper records
- Assists with the follow up of incomplete records to physicians and their office
- Understanding of TDO (Terminal Digit Order)
- Understanding and maintaining workflow queues and DCS
- Ability to handle/use scanner understand how it functions and how to maintain the scanner
Daily Processing of Medical Records 30%
- Retrieves patient discharge charts from patient care area
- Tracks inpatient/outpatient charts received and identify any charts not obtained
- Notification to direct supervisor when charts are not received
- Assures that patient identifying information is on each page of the paper record
- Maintains documentation in medical record in the correct sequence per policy
- Assists with Prepping within 24 hours Scanning within 48 hours
- Understands process of all documents that cannot be scanned. Cataloging and maintaining records.
- General/Daily Understanding of work flows 10%
- Answer telephones assist callers
- Assist customers that come to HIM department
- Good understanding of release of information
- Knowledge of ADT within Paragon Medical Records
- Understands how to use Paragon Medical Records MPF and Physician Workstation
- Ability to retrieve and file back paper records as needed
- Performs other duties as assigned and requested
- Maintains an organized work area
- Adheres to organizations Standards of Conduct
- Good understanding of HIPAA and protect/security of PHI
Qualifications
Required
- Six months to one years experience in customer service and clerical.
- Hold or receive required Los Angeles County Fire Card within no more than 90 days of hire or renewal.
- Maintains Skill Assessments annually as required by hospital
- Ability to access and input/ retrieve information from a computer.
- Good communication skills both oral and written.
- Ability to exhibit a courteous and professional manner.
- Demonstrates ability to ensure that strict confidentiality guidelines are adhered to.
- Ability to comprehend and follow instructions to make decisions and to prioritize tasks to meet appropriate deadlines.
- Ability to focus and maintain attention to the performance of tasks despite interruptions.
- Ability to provide very accurate work with attention to detail.
- Ability to perform most tasks independently with moderate supervision.
- Ability to use office equipment includes but not limited to a calculator computers computer printers fax machine and photocopier equipment.
