Outpatient Family / Primary Care
Full-Time Federally Qualified Health Center (FQHC)
A large mission-driven Federally Qualified Health Center (FQHC) is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its outpatient primary care team. This is an excellent opportunity to practice comprehensive family medicine in a collaborative well-supported environment focused on serving diverse and underserved communities.
Position Highlights
100% outpatient primary care
Adult and pediatric patient population
Team-based care model with physicians APPs nursing behavioral health and care coordinators
Mission-driven organization with strong leadership and clinical support
Predictable weekday schedule (no nights or weekends)
Responsibilities
Provide high-quality outpatient primary care services to patients across the lifespan
Diagnose treat and manage acute and chronic conditions
Perform routine physical exams preventive care and health screenings
Develop and implement treatment plans and patient education
Document care in the electronic medical record (EMR)
Collaborate with physicians and interdisciplinary care teams
Participate in quality improvement initiatives and population health efforts
Qualifications
Licensed Nurse Practitioner or Physician Assistant
Board certified or board eligible
Active or eligible state license
Primary care or family medicine experience preferred
Commitment to community-based and underserved care
Compensation & Benefits
Competitive salary range: $120000 $140000
