Community Health Services is seeking a dedicated Family Medicine Physician to join our collaborative team-based primary care environment. This role offers the opportunity to provide comprehensive patient-centered care across the lifespan while working alongside experienced clinical and support staff in a mission-driven organization.
The Family Medicine Physician delivers preventive care chronic disease management and acute care services while actively participating in population health initiatives and coordinated care efforts.
Required Qualifications:
- MD or DO from an accredited medical school
- Board Certification (or Board Eligibility) in Family Medicine
- Active Connecticut physician license (or eligibility)
- DEA registration and state controlled substance registration (or eligibility)
- Commitment to delivering high-quality efficient and compassionate patient care
- Strong ability to work collaboratively in a multidisciplinary team-based setting
- Demonstrated experience providing culturally competent care to diverse patient populations
- Excellent communication skills with patients colleagues and leadership
- Proven ability to maintain timely thorough and accurate clinical documentation
- Comfort using electronic health records and clinical technology
- Sound clinical judgment and an evidence-based approach to care
- A proactive solutions-oriented mindset with a strong sense of accountability
All candidates must successfully complete a pre-employment background check and drug screen.
Preferred Qualifications:
- Three (3) or more years of experience in an ambulatory or urgent care setting
- Experience working with underserved or socioeconomically diverse populations
- Bilingual (Spanish/English) candidates strongly preferred
