Registered Nurse (RN) - Diabetes Educator - St. Francis Millennium Medical Office Building
Job Summary
The RN Certified Diabetes Educator (CDE) is a clinician prepared to manage the diabetes population at specific clinical site(s) to support long-term diabetes health. Position responsibilities include identifying the population at risk for diabetes complications; intervening to close care gaps through active collaboration with providers; implementing individual and group diabetes self-management education/training (DSME/T); supporting patients with diabetes (PWD) in changing self-care practices to achieve diabetes health; and participating in quality initiatives of the BSPDH to improve the DSME/T program.
In addition provide diabetes self-management education and support in an American Diabetes Association recognized program in accordance with the Standards of Medical Care in Diabetes and the National Standards. The RN Diabetes Educator teaches persons with diabetes inpatient and outpatient in a group or individual setting and utilizes established principles of teaching learning theory and lifestyle counseling to successfully direct behavior change. Job also includes additional tasks associated with providing and promoting a quality diabetes education service including but not limited to: scheduling follow up EMR documentation performance improvement projects data collection reporting and analysis and participating in community/hospital events and committees as well as supporting Mercy physician/endocrinology practices as needed. Performs other duties as assigned to meet needs of the department.
Essential Functions:
Completes administrative tasks such as makes initial and follow up calls to referred patients schedules patients in cadence as well as checks patients in charges and verifies charges contacts physician offices and hospital nursing units as needed manages the referral work queue. Educator creates and sends letters and records and manages all processes in an organized and accurate manner. Completes additional operations-related tasks as needed.
Collects assessment data in a systematic and organized fashion from the person with diabetes family members existing medical records and referring healthcare providers. Assesses patient current health medical history nutrition practices physical activity medications literacy behaviors psychosocial/social/ cultural factors and access to healthcare resources. Documents assessment interventions and evaluation of outpatient and inpatient care in the electronic health record according to policy.
Works with patients to identify mutually acceptable DSMES outcomes which provide direction for the patient and are used to evaluate progress toward individual program institutional and community level goals. Expresses outcomes in clearly defined measurable terms. Provides outpatient diabetes self-management education and support (DSMES) in group and individual settings according to the defined plan and desired outcomes at all multi-sites as needed. Is responsible for following the Standards of Diabetes Care and the National Standards of the American Diabetes Association for Education Recognized Programs when planning and providing education. Teaches self-management principles and skills. Provides follow up care. Provides inpatient survival skills education. Collaborates with hospital nurses and medical team to provide home-going diabetes education. Participates in the ongoing development implementation and evaluation of all regional education services including grant-funded services.
Evaluates the quality and outcomes of DSMES for each client (individual outcomes measures) and for the program (aggregate outcome measures) according to the standards defined by AADE (American Association of Diabetes Educators) and ADA (American Diabetes Association). Communicates findings and trends with referring providers and assists with finding and using resources for diabetes management. Supports and promotes community-based programs. Participates in all quality improvement projects data collection analysis and evaluation on an ongoing basis.
Collaborates with various healthcare professionals and departments. Participates in numerous Mercy Health and community activities and events (including volunteer) to promote diabetes self-management as well as promote the community hospital community and departmental strategies. Provides MH staff education and gives presentations. Complies with all licensure and continuing education requirements of an Education Recognized Program (ERP).

Education:
Bachelor of Science Nursing (preferred not required)
Graduated from accredited School of Nursing
Licensure:
Current RN license in State of South Carolina
Certification:
Certified Diabetes Educator or earn CDE within 6 months of hire
Minimum 3 years
Competence in delivering diabetes self-management training individually and in groups using nationally recognized self-care behavior framework
Competence in educating and using patient tools including but not limited to blood glucose monitoring and continuous glucose monitoring systems required within one year
Strong written and verbal communication skills to include facilitation and public speaking
Experience:
Teaching experience of adult learners 1:1 and in group setting is required. Excellent interpersonal and customer service skills required. Must have excellent verbal and written communication skills.
Preferred: Prior experience as a diabetic educator and/or prior experience in group education and individual counseling as well as background knowledge with Insulin Pump and CGM use.
Minimum Years and Type of Experience: Minimum of 4000 hours (approximately 2 years full time) experience in clinical care delivery
Experience and use of an EMR. Conceptual thinking (sees patterns).
Able to prioritize responsibilities according to department activities and needs. Teamwork & Cooperation. Critical thinking skills. Knowledge of diabetes. Effective communication orally and verbally in a timely manner with all internal and external customers (patients/clients/physicians/families/staff etc.); effective teaching skills- clearly according to curriculum and with enthusiasm; Microsoft office skills using word excel and PowerPoint
