Band 6 Occupational Therapist Community Services
Location: Bristol
Contract: Temporary
Hours: 4-5 days per week totaling 37.5 hours
Our client is seeking an experienced Band 6 Occupational Therapist to join their team in Bristol. This role involves working across acute and community settings supporting patients with complex needs and ensuring safe timely discharges.
Key Responsibilities:
- Support discharge planning prevent admissions and manage frailty cases.
- Provide interventions for patients with falls risk neuro and orthopaedic needs.
- Work collaboratively with the multidisciplinary team to deliver patient-centered care.
- Promote independence and functional recovery through evidence-based practice.
Requirements:
- HCPC registration as an Occupational Therapist.
- Proven experience in acute and/or community settings.
- Valid driving licence and access to a car (essential for community work).
- Strong understanding of discharge pathways and admission avoidance.
- Excellent communication and teamwork skills.
