Social Worker PNE LCSW-Palliative Care Part Time Days

**This is a Part Time position at 20 hours per week for Palliative Care located at Lake Forest Hospital.**

The Social Worker reflects the mission vision and values of NM adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.

The Social Worker is responsible for providing psychosocial counseling and intervention services to patients who request their services or any patient referred by a provider. The social worker advocates for the patient and family and assists in outpatient continuity of care.

Responsibilities:

• Completes a thorough psychosocial assessment for new patients.
• Utilizes critical thinking to identify and screen high risk social cases.
• Provides pre-treatment planning psychosocial counseling/interventions health education financial counseling referrals and discharge/transition planning.
• Collaborates with team other ancillary departments case management staff and social service agency providers to develop a care plan.
• Provides resources including education materials to patients and families in the areas of financial assistance transportation scheduling and community resources to support the care plan.
• Supports department initiatives (attends case conferences participates in quality improvement).
• May perform other duties as assigned.

Qualifications :

Required:

• Masters degree.
• Licensed Clinical Social Worker (LCSW) in the state of Illinois.
• Zero (0) to two (2) years experience in a relevant role.
