The Nurse at Old Town Clinic role works collaboratively as a member of a multi-disciplinary team providing medical behavioral and substance use treatment services and care coordination for complex medically vulnerable adults. This role will float between primary care clinic RN duties on site triage and phone triage.
Schedule: Monday Wednesday Thursday Friday (7:30am - 5:00pm)
Location:Old Town Clinic (727 W Burnside Street Portland OR 97209)
Compensation: $44.05 - $57.27 per hour compensation will be decided based on candidates experience and internal equity.
MINIMUM QUALIFICATIONS
- Successful completion of a course of accredited study that leads to the ADN or BSN degree in Nursing from OSBN or State Board approved course.
- Current RN license in the State of Oregon.
- BSN or QMHP preferred.
- Must meet CCC privileging requirements as required by FTCA.
- Experience working with populations experiencing significant gaps in social determinants of health preferred.
- Current BLS certification prior to start.
- Must generally have the ability to walk bend stoop kneel squat twist reach pull and lift heavy objects and climb stairs with reasonable or no accommodation.
- Ability to adhere to Central City Concerns drug-free workplace which encourages a safe healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not in the workplace unlawfully manufacture distribute dispense possess or use a controlled substance or alcohol.
- Must pass a pre-employment drug screen TB test and background check. For work at the Blackburn Center this includes clearance by the DHS Background Check Unit.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Clinic Duties
- Provide nursing assessment intervention education and follow up in a primary care setting that integrates substance use and mental health treatment.
- Collaborate with the interdisciplinary team to provide integrated patient care.
- Primary care duties include wound care and administration of medications safely and per organizational protocols.
- Respond to urgent in-person and telephone triage needs and client follow up.
- Provide clinical consultation to medical assistants and other clinical and support staff.
- Collaborate with team members across Central City Concern and care providers outside of the organization.
- Participate in compliance quality assurance and quality improvement activities as directed.
- Adhere to all state and federal privacy regulations including HIPAA and 42 CFR Part 2 and to CCC policies and agreements regarding confidentiality privacy and security. Support compliance with all privacy and security requirements pursuant to community partners and outside providers patient confidentiality agreements including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department as well as to your supervisor or their designee.
- Other patient care duties as assigned and within scope.
Triage Duties
- Assess and triage patients in-person primarily and on the phone as needed.
- Provide nursing assessment and service matching according to clients health goals presenting needs while balancing clinic schedule and services available.
- Closely communicate with team members about clients needs and in consideration with clinician capacity.
- Requires flexibility with assignments as clinic needs arise.
Phone Triage and Desktop Duties
- Telephone based role providing triage and problem solving for clients calling into clinic with symptoms or health concerns .
- Responsible for follow up and care coordination as requested by care team via phone or electronic health record.
SKILLS AND ABILITIES
- Ability to consider the impacts and outcomes for underserved communities during decision-making processes.
- Experience with care for individuals with complex substance use and mental health backgrounds using trauma informed care principles and de-escalation techniques.
- Ability to be flexible and to coordinate and relate effectively with members of several interdisciplinary teams participants supervisors and co-workers.
- Ability to multi-task and remain calm in a busy and stressful environment.
- Ability to work independently prioritize the workload problem-solve and complete tasks in a timely fashion.
- Ability to maintain composure and work objectively with participants who may be verbally aggressive and/or substance affected.
- Working knowledge of principles practices and procedures of ambulatory care.
