Position Purpose:
The Palliative Care RN Registered Nurse is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation.
What You Will Do:
- Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation
- Motivates/supports employees for exemplary performance on internal and external customer service productivity and accuracy
- Provides statistical/budgetary data to appropriate Palliative care program
- Works collaboratively with all departments to ensure execution of quality delivery of services
- Ensures that stakeholders are satisfied with services provided; improvement plan developed when necessary
- Develops tracking outcomes of enrolled patients and ensures information is available for review
- A commitment to quality palliative care provision and transition to hospice when appropriate
- Exercises sound clinical judgment in oversight of Palliative Care Program
- Knows understands incorporates and demonstrates the Trinity Health Mission Vision and Values in behaviors practices and decisions.
Minimum Qualifications:
- Graduate of an approved nursing education program.
- Current Registered Nurse licensure in the state of practice.
- CPR certification
- Hospice/Palliative Care certification preferred.
- Home care experience strong clinical knowledge base Hospice experience computer skills strong interpersonal and organizational skills
- Palliative Care experience demonstrate ability to use problem solving critical thinking and priority setting skills
- Must be able to work independently and on a team
- Ability to perform diversified duties with time limitations
- Ability to develop coordinate and evaluate programs in Palliative care and end-of-life care
