Description
AMHC is seeking a compassionate organized and motivated Full Time Adult Behavioral Health Home Coordinator / Community Integration Specialist to support adults living with long-term psychiatric disabilities in achieving stability independence and wellness within their communities.
Key Responsibilities
- Build strong therapeutic relationships including engaging hard-to-reach individuals
- Assist clients in identifying personal wellness goals and developing skills to achieve them
- Coordinate and manage services related to each clients Plan of Care (Section 92) or Individualized Support Plan (Section 17)
- Provide supportive counseling decision-making assistance and crisis intervention as needed
- Develop and facilitate skill-building and psychoeducational groups
- Coordinate care with physical and behavioral health providers
- Assist clients in accessing community resources including accompanying clients when appropriate
- Support families and significant others (with client authorization)
- Maintain accurate clinical and administrative documentation
- Participate in team meetings supervision and staff development activities

- Ability to work effectively with individuals from diverse backgrounds
- Comfort working both independently and as part of a team
- Flexibility to meet client needs including occasional extended hours
- Commitment to confidentiality ethics and professional standards
Work Environment & Requirements
- Work occurs in office community and client home settings
- Regular travel within the service area is required
- Ability to sit stand walk and lift up to 25 lbs
- Must be able to use standard office equipment and computer systems

Requirements
- The Adult Behavioral Health Home Coordinator/Community Integration Counselor requires an associate degree or equivalent credits in a Human Services field. Bachelors degree preferred.
- Must be certified at the full MHRT-C or provisional level. Use of personal vehicle and active drivers license is required.
