Position Overview
We are seeking a licensed Physical Therapist to provide high-quality patient-centered care across clinical and customer-site settings. This role focuses on improving mobility function and overall quality of life through individualized treatment plans.

Key Responsibilities

Evaluate patients and develop customized treatment plans
Deliver evidence-based physical therapy interventions
Document patient progress and adjust care plans as needed
Educate patients and caregivers on exercises and injury prevention
Travel to customer or partner sites approximately 30 40% of the time
Collaborate with interdisciplinary teams to support patient outcomes

Qualifications

Licensed Physical Therapist in the state of Indiana
Doctor of Physical Therapy (DPT) preferred
Minimum of 1 year of clinical experience
Strong communication and time-management skills
Ability and willingness to travel to customer sites
