INTAKE OFFICER
Position Objective:
The Intake Officer works with clients care coordinators and service providers to streamline service provision to achieve the best outcome. The Intake Officer shall utilize their extensive knowledge about Corryong Health to assist clients in identifying the appropriate community-based services to meet their needs and achieve their goals.
Primary Responsibilities:
- Interact effectively with all care providers and gain a thorough knowledge and a clear understanding of program eligibility for internal and relevant external services
- Identify clients with chronic and/or complex needs at point of entry regardless of their original referral point
- Standardise screening tools and implement well-developed systems and processes to support the flow of intake and referral management
- Timely completion of the intake process to ensure continuum of care through the appropriate pathway
- Facilitate access for people to Corryong Health services that will support their capacity to pursue their client goals
- Undertake appropriate initial needs identification with early consent/ agreement between clients and intake worker/s and ensure clients are aware of their rights and responsibilities
- Work collaboratively with the broader community based multi-disciplinary team
- Strong time management skills with the ability to ensure data is regularly updated and client appointments are efficiently scheduled including home visits in-office consultations and triage sessions
Selection criteria:
Essential:
- Excellent written and oral communication skills and demonstrated ability to meet state and commonwealth reporting requirements
- Knowledge of relevant legislation including privacy confidentiality and conflict of interest
- Demonstrated ability to liaise consult and negotiate with a wide range of stakeholders including internal service providers and external organisations/ agencies
- Demonstrated commitment to promoting and providing excellent customer service with a client centered focus and to advocate for clients
- Demonstrated ability to understand the funding streams supporting services available within Corryong Health and associated client eligibility
- Demonstrated drive commitment and resilience within a framework that highly values personal and staff wellbeing
- Experience and competence in the use of the Microsoft Office Suite
Desirable:
- Previous experience in an Intake role
- Experience in Community Health or Hospital setting
- Understanding of client centered and integrated care models
- Proven understanding of the role and value of healthcare organisations in a rural environment
- Valid Victorian or NSW Drivers License
