The Team Lead is responsible for the day-to-day clinical operation and patient flow of the one-Link program including:
- Acting as the point of contact for referral flow to the program including liaising with referral sources and partner mental health and addiction services.
- Acts as a clinical resource for problem solving complex cases and finding service coordination options in collaboration with health service partners.
- Supporting the clerical staff with prioritization of transcription patient visit bookings re-scheduling and workflow.
- Collaborate with internal and external stakeholders to ensure ongoing evaluation and continued streamlining of coordinated access and updating processes as needed to facilitate timely access to care.
- Supporting quality improvements and evidence-based practice for all members of the team.
- Creates an environment that promotes collaborative teamwork clinical inquiry innovative thinking and greater autonomy in clinical decision-making.
- First point of contact for investigation into complaints from patients/referral sources or incident reports and escalates needs to manager.
- Provides clinical leadership through onboarding and orientation coaching education and mentoring to all staff in the program to meet best practice standards in alignment with program and organizational expectations.
- Promptly follows up on any patient complaints or incidents utilizing each complaint/incident as an opportunity for CQI. Completes incident report and utilizes the support of the manager as necessary to reach a satisfactory resolution.
Qualifications :

- Bachelors degree from a social service/ healthcare program
- Masters degree preferred
- Regulated Healthcare Professional (Registered Nurse Social Work Occupational Therapy) preferred
- Minimum of 5-7 years mental health and addiction service delivery experience in hospital and community setting
- Minimum 3 years working experience in Mental Health and Addiction coordinated access model such as one-Link
- Minimum of 3-5 years participating and/or leading clinical quality improvement projects relevant to mental health and addiction service
- Minimum 3 years mentorship supervision or teaching experience/qualifications
- Significant experience in leading a knowledge translation project or change management initiatives to improve care/service delivery
- Able to professionally and articulately represent the program at various internal and external committees.
- Able to demonstrate the ability to work across multiple sectors and create positive relationships
- Demonstrated skills in change management and conflict resolution.
- Demonstrated understanding of mental health and addiction system and services in the region
- Proven ability to provide conflict resolution through listening feedback and coaching.
- Ability to travel by car between sites and to meetings (2-5 times per month).
Additional Information :
Hours: Monday to Sunday; Various rotating 7.5 hour shifts and every other weekend (Hours are subject to change based on operational needs)
Salary: $53.026 - $66.308 per hour
