We are currently looking for a community based Enrolled Nurse - Care Partner to join our Geelong team. The role:
This position is part time with a view to increasing to full time. The role involves supporting a case load of participants to assist them to coordinate their care and services under the Support At Home program so they can remain living independently in their own homes. It is mostly office based but there is some travelling between participants homes to provide individual support and assessment to participants and their families. It may involve some nursing duties such as wound management and coordination of clinical supports under the supervision of the Registered Nurse team.
Duties and responsibilities may include:
- Working with or under the guidance of Registered Nurses Allied Health professionals and medical practitioners
- Undertake comprehensive participant care planning by identifying and assessing participant needs goals preferences and existing supports.
- Conduct non-clinical participant assessments and complete documentation in accordance with Program Guidelines.
- Monitor and review participants budgets and management of their budget in accordance with Support at Home guidelines.
- Ensure efficient service coordination by communication and coordination with workers involved in the delivery of services and with the participant and their family and informal carers.
- Ensure coordination teams have suitably trained and skilled staff to deliver services in line with the participants care needs and care plan.
- Ensure appropriate information is provided and explained to the participant at all times regarding the service agreement including the participant handbook and rights under Support at Home.
- Monitor review and evaluate participants ongoing care and changing needs ensuring care plans are regularly reviewed and updated.
- Monitor changing care needs and escalate new or emerging clinical risks to the Clinical Care Partner for review.
- Where applicable develop strategies which will address the identified needs of the participant in consultation with their supporter/s where appropriate in accordance with funding guidelines.
- Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.
- Monitor review and evaluate ongoing care discussions case conferencing with relevant team members and/or external service providers.
- Ensuring that participants changing needs and emerging risks are monitored and reviewed.
- Support participants to make informed decisions and integrating re-ablement approaches.
- Medication Administration and Complex Clinical Care e. Palliative Care Epilepsy Bowel Care/ Suppositories Catheter Care Stoma Care Oxygen and more
- Monitoring and communicating on the customers wellbeing and reporting on progress of customers
- Ensuring that all services are provided in a caring and respectful way in accordance with Just Better Cares policy and procedures legislative requirements and program specific guidelines (i.e. Support At Home program)
- Provide coaching and guidance to Community Support Professionals and Support Staff/Case Managers as required
- Demonstrating a commitment to best practice and evidence of continued professional development
- Participating in staff meetings and attending training opportunities when required
What you will need:
- Previous experience supervising supporting and training staff.
- Ability to work flexibly to respond to changing participants needs.
- Ability to coordinate services according to relevant program guidelines.
- Experience of working in a multi-disciplinary team.
- Ability to develop and maintain links and supportive networks with specialist services.
- Knowledge of continuous quality improvement principles.
- Excellent people problem solving and conflict management skills.
- Ability to work as part of a team and maintain good working relationships
- Experience maintaining computerised participant information systems.
- High level administrative and organisational skills.
- High level of written and verbal communication skills.
- Demonstrated ability to contribute positively to the team.
- Current Enrolled Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Current Federal Police Check & Working with Children Check
- Current CPR / First Aid Certificate
- Current Australian Drivers Licence
- Reliable registered and insured vehicle (comprehensive insurance is preferred)
- Smartphone with internet access
- Proof ofof Vaccination (where relevant) i.e. COVID-19 vaccination certificate
Desirable:
- Previous experience in the community care or health/aged care sector
- Previous community nursing experience
- Previous experience in a Case Management role/Care Partner role.
