About the Role
An experienced Registered Nurse is sought for a Clinical Care Partner role supporting participant-directed care across private NDIS and community services.
The role combines clinical care coordination and stakeholder engagement ensuring participants receive safe well-planned and responsive services aligned with their needs.
Key Responsibilities
- Conduct onboarding and comprehensive clinical assessments for new participants
- Develop review and update care plans and related documentation
- Provide clinical nursing services when required (e.g. wound care cannulations)
- Review End of Shift Reports and follow up on identified care issues
- Conduct regular participant visits (every 36 months or as required)
- Provide guidance supervision and feedback to support workers
- Liaise with participants families allied health professionals and referral partners
- Participate in clinical meetings audits and continuous improvement initiatives
- Support service growth and achievement of billable hour targets
- Participate in one weekend or after-hours RN shift per month (as required)
Requirements
About You
- Bachelor of Nursing with current AHPRA registration
- Minimum 2 years post-registration nursing experience
- Experience in home care NDIS private or community health (highly regarded)
- Strong clinical judgement communication and interpersonal skills
- Comfortable working independently while collaborating within a small team
- Confident using clinical and rostering systems
- Reliable organised and solutions-focused

- Working With Children Check
- Full Australian working rights
