We are currently recruiting a Medical Social Worker for a leading nursing home in Dublin Ireland.
Hours of work: 37.5 hours per week
Key Responsibilities
Provide a comprehensive social work and counselling service to patients and their families ensuring holistic person-centred care.
Demonstrate excellent communication and interpersonal skills to build effective collaborative working relationships within the multidisciplinary team.
Develop robust individualised patient care and discharge plans in partnership with patients families and the multidisciplinary team to ensure safe and effective transfer to the community.
Apply and promote the social model of care supporting and educating colleagues on its role and how it complements the medical model in patient care.
Maintain a strong focus on the primacy of the patient in all aspects of practice upholding human rights dignity integrity and consent-led interventions.
Manage a diverse caseload including complex urgent emergency priority routine episodic and acute cases in line with service needs.
Foster strong multidisciplinary professional relationships that support collaborative person-centred practice across the service.
Participate fully in performance management processes including regular review and appraisal as directed by the Director of Clinical Services and the Principal Social Worker.
Practice in accordance with CORU Code of Ethics and Standards hospital policies and procedures and all relevant health and safety regulations.
Requirements
Hold a recognised third-level qualification in Social Work.
Maintain current registration with CORU.
Possess a relevant postgraduate qualification (desirable).
