Responsibilities
Conduct psychosocial assessments for patients and families.
Identify emotional social financial and environmental needs affecting patient care.
Develop personalised care and support plans.
Provide crisis intervention when required.
Coordinate safe and effective discharge planning.
Arrange community supports such as home care and rehabilitation services.
Offer counselling and emotional support to patients and families.
Support end-of-life care discussions and bereavement needs.
Advocate for patients rights and assist with accessing benefits and social supports.
Work closely with multidisciplinary teams to deliver holistic patient care.
Identify and report safeguarding or protection concerns for vulnerable individuals.
Link patients with appropriate community resources and support organisations.
Maintain accurate documentation and prepare reports as needed.
Requirements
- Must be registered with the Social Workers Registration Board at CORU and maintain an active annual registration
- Minimum of one year post-qualification experience.
