Position Summary:
Assesses and plans interventions to help patients cope with social emotional economic and environmental problems.
Essential Functions:
Interviews and assesses patients and/or patients family caregivers and/or legal representatives.
Determines prioritizes provides and/or arranges for needed internal and external services/interventions.
Participate in case reviews to evaluate case management and progress.
Consults with healthcare team members to promote monitor and evaluate compliance with patients treatment plan.
Assists with discharge planning and processes.
Identifies appropriate resources including transportation housing healthcare and social/spiritual services and provides referrals as part of the discharge plan.
Supervises and/or trains new staff students and interns.
Provides individual family and group therapy.
Required Skills:
Requires critical thinking skills decisive judgment and the ability to work with minimal supervision.
Must be able to work in a stressful environment and take appropriate action
Education/ Certification
Bachelors required; Masters preferred Licenses
Accepting (CSW LPCA LCSW RN BSN)
Board Certified Social Worker or related field within 6 months of hire Certifications:
Handle With Care within 30 days of hire
