Clinical Support Representative (Patient Care Coordinator) Overview:
  • Competitive Compensation:$23-26/hr
  • Office Hours: 8:30 AM - 5:00 PM (Mon - Fri)
  • Comprehensive Benefit Package including PTO holiday pay medical/dental/vision insurance and a 401(k) plan
Your Role and Responsibilities as aClinical Support Representative (Patient Care Coordinator):
  • Manage and coordinate patient and staffing scheduling
  • Answer inbound calls and schedule accordingly: intake start of care recerts transfer orders and discharge
  • Call referral sources to confirm next day actions
  • Follow up with all hospitalized patients and documents in the patient EMR daily via communication notes
  • Works in conjunction with clinical staff to ensure protocols and processes are being followed
  • Actively participates as a part of a high-performance work team to drive and manage change to deliver exceptional patient/client service.
Qualifications for aClinical Support Representative (Patient Care Coordinator):
  • High School Diploma or GED
  • Minimum 2-3 year of experience in healthcare scheduling or intake
  • Proven organizational skills and detail orientation
  • Ability to multi-task excellent communication skills and a customer service mindset
  • Experience with MatrixCare or similar healthcare-related scheduling system(s)
