About the role
Support delivery of therapy and care services for clients in the Transitional Aged Care Program working within a friendly and supportive team environment focused on delivering person-centred care
Work collaboratively with the multidisciplinary team to provide slow-stream rehabilitation to recently discharged hospital patients
Perform delegated personal care domestic assistance and community-based rehabilitation tasks as per care plans
Carry out exercise and therapy programs under supervision of Senior Allied Health staff
Accurately document health information and communicate effectively with patients and healthcare team members
About you
Hold relevant qualifications such as a Certificate III or IV in Allied Health Assistance be currently working towards an allied health university degree or possess an international allied health qualification along with a current unrestricted NSW Class C drivers licence.
Demonstrated effective interpersonal written and verbal communication skills with the ability to communicate with a range of people
Demonstrated experience working with older people in a community and/or rehabilitation setting
Demonstrated ability to prioritise workload meet deadlines and complete activities within scope of practice seeking assistance when required from the supervising Allied Health Professional
Demonstrated computer literacy skills and knowledge of Microsoft Office and email applications
