Job Summary

Community Support Team

The Team Lead Case Therapist II reflects the mission vision and values of NM adheres to the organizations Code of Ethics and Corporate Compliance Program and complies with all relevant policies procedures guidelines and all other regulatory and accreditation standards.

This role is responsible for providing intensive case management services to individuals with chronic and/or persistent mental illness or co-occurring disorders.

Specific responsibilities may include but not be limited to:

  • Assisting clients in engaging in needed services assisting in finding and utilizing community resources applying for and re-determining benefits assistance assisting clients in having basic necessities such as food housing and safety resources.

Caseload: This is a team-based approach. The team shares the caseload but each team member is individually assigned specific clients for consistent follow-up and support.

Population: Clients have higher needs and are at higher risk of crisis and instability than those clients engaged in traditional case management services.

Schedule:

40 hours/week Monday-Friday 8:30-5:00

*No overnight or weekend hours.


Qualifications :

Required:

  • Experience and training in assessment individual family and group counseling
  • Masters Degree with LSW LCSW LPC or LCPC required or eligible for licensure
  • Masters degree in social work psychology counseling or related field

Preferred:

  • 3-5 years in intensive treatment setting and /or mental health setting.
  • CADC Licensure
  • LIC CPC
  • LIC LCSW
  • LIC MSW
