Job Title: Licensed Practical Nurse (LPN) Long-Term Care (LTC)
Location: Hood River OR 97031
Contract Duration: 3 Months (Extendable)
Shift: Night Shift 312 hours
Weekly Hours: 36 hours/week

Position Overview
A long-term care facility in Hood River OR is seeking an experienced Licensed Practical Nurse (LPN) for a night-shift contract role. The LPN will provide high-quality nursing care to residents while supporting overall clinical operations in a skilled nursing environment.
Key Responsibilities
Deliver high-quality nursing care including ADL support mobility assistance and routine health monitoring.
Administer medications and treatments per provider orders (wound care injections catheter care etc.).
Monitor and document resident conditions; promptly communicate major changes to RN or provider.
Complete accurate charting in EHR systems including MAR/TAR and shift documentation.
Participate in care plan updates and implement individualized resident treatment plans.
Perform clinical procedures including vital signs blood glucose monitoring oxygen therapy and G-tube feedings (if applicable).
Ensure resident safety through infection control fall prevention and continuous monitoring.
Offer emotional support to residents and families promoting dignity and comfort.
Supervise and delegate duties to CNAs ensuring compliance with facility standards.
Collaborate with interdisciplinary teams (PT/OT social workers physicians) to support holistic care.
Respond to medical emergencies following facility protocols.
Maintain compliance with state regulations LTC standards and facility policies.
Required Licenses & Certifications
Active LPN license (state-appropriate or compact as applicable)
BLS certification (AHA)
COVID-19 vaccination card
