Job Title: Social Worker - Geriatric Ambulatory Care
The Mount Sinai Hospital is a 1134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical emotional and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907 the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education professional development and extra-curricular opportunities.
Mount Sinai Health Partners (MSHP) is the management services organization developed to implement the Health Systems population health initiatives: programs of care that enhance the individual patients experience and health status improve the health of patient populations and reduce utilization and healthcare costs. MSHP provides the infrastructure and machinery to support population health initiatives particularly preventive care through care coordination information technology workflow optimization and quality reporting. The Department of Social Work Services is positioned to take a lead role in the development of MSHP by providing transitional and longitudinal care coordination services.
The Martha Stewart Center for Living practice embedded social worker is an integral member of an interdisciplinary team providing episodic care to geriatric patients with a range of psychosocial needs. The social worker conducts brief biopsychosocial assessments of patients and their caregivers and based o that assessment works to connect the patient and caregiver to resources to address the identified SDH needs. Reasons for referral include: mental health issues such as depression and dementia; patient safety; functional issues requiring homecare to assist with ADLs/IADLs; medication and medical regimen adherence; patient access to care including transportation; insurance/financial issues; caregiver/family stress; end of life issues; and elder abuse among others. Types of supportive intervention include: face-to-face meetings; follow-up phone contact supportive counseling; patient advocacy; referrals for community services; engagement with insurance providers; APS referrals; provision of resources; referral for ongoing case management.
This position is ONSITE and is based at Martha Stewart Center for Living (MSCL) the Mount Sinai Hospital Campus and focuses on geriatric patients and their families.
Compensation Statement
The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $85280.00 - $97760.36 Annually. Actual salaries depend on a variety of factors including experience education and operational need. The salary range or contractual rate listed does not include bonuses/incentive differential pay or other forms of compensation or benefits.
