Job Summary
Duties:
- Provide social work services to patients and their families in a medical setting
- Conduct assessments to determine the psycho-social needs of patients
- Develop and implement care plans based on patient needs and goals
- Collaborate with healthcare professionals to coordinate and facilitate patient care
- Assist patients and families in accessing community resources and support services
- Provide counseling and emotional support to patients and families
- Advocate for patients rights and ensure they receive appropriate care
- Document patient interactions and maintain accurate records
- Supervise and document limitedlicense medical social workers during counseling sessions
- Review and meet with LMSW to discuss their progress
Experience:
- Masters degree in Social Work or related field
- Previous experience working as a Medical social worker preferred
- Knowledge of medical coding research and documentation practices
- undergraduate degree
