Key responsibilities include conducting comprehensive assessments developing and updating individualized care plans and delivering timely safe medication administration in accordance with facility policy and regulatory requirements. Youll perform wound and skin care coordinate IV therapy where appropriate monitor for acute changes and respond with clinically appropriate interventions. The role emphasizes patient and family education falls prevention infection control and documentation that supports continuity of care. Youll collaborate with a multidisciplinary teamphysicians social workers therapists and nursing assistantsto ensure transitions of care are seamless and outcomes improve steadily.
