About the role
The Homeless Health Service (HHS) is an outreach-based healthcare service dedicated to improving access to care for people experiencing homelessness. The team meets people where they areproviding healthcare in the places people sleep live and spend time throughout the day.
The Mental Health Clinician works within the Homeless Outreach Team (HOT) which manages all intake and referrals into the HHS. HOT delivers multiple weekly outreach clinics multi-agency community hubs and street-based patrols across the inner city including through the Sister Clare Richardson Mobile Clinic.
The HOT Mental Health Clinician is responsible for intake assessment brief intervention care planning advocacy and coordination of referrals. The role works closely with local Specialist Homelessness Services and Homes NSW to support timely access to housing and ongoing care.
This is an autonomous role that operates within a strengths-based trauma-informed and harm-minimisation framework responding to each persons cultural needs and holistic health priorities.
About you
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse Psychologist or Occupational Therapist or eligibility for membership with the Australian Association of Social Workers (AASW).
Current unrestricted NSW Drivers Licence.
Demonstrated ability to work effectively within a multidisciplinary team and collaborate across a wide range of internal and external stakeholders.
