Staff Nurse Diabetic Day Centre Tallaght University Hospital
37.5 hours per week
Reference Number: 12.006*(2025)Are you a dedicated and passionate Staff Nurse looking to make a change in your career and gain more experience We are currently looking for a Staff Nurse to transfer to our Diabetic Day Centre where there is one post of 37.5 hours available.
Qualifications & Experience
Must have:
- Be a current Staff Nurse employed by Tallaght University Hospital
- Be a Member of the General Division of the active register of the Nursing and Midwifery board of Ireland.
- Demonstrate evidence of continuing professional development at the appropriate level.
- Have excellent communication and interpersonal skills.
- Experience in acute care sector managing patients with diabetes.
- Have a clinical and administrative capability to properly discharge the functions of the role.
- Have an interest in Diabetes with experience in the area and a commitment to further studies.
Roles and responsibilities of the post (in conjunction with CNS and ANP colleagues):
- Provide structured education to patients who are newly diagnosed with T1 and T2 diabetes with regard to diagnosis prognosis planned treatments and side effects and follow up.
- Assess patients understanding of their diagnosis and its implications
- Communicate with patients families and friends assess needs and provide relevant support information education and advice.
- Provides educational and consultative sessions for both the patient and their family members to discuss any ongoing concerns or for education regarding their condition or their plan of care.
- Ensure that patient has a follow up appointment to enhance their diabetes knowledge
- Provide psychological emotional and informational support to patients and their families.
- Co-ordinate appointments with other members of the multidisciplinary team including the dietician diabetes medical team Podiatry service and psychologist
- Refer patients for retinal screening
- Provide education to use Continuous glucose monitoring apply for funding connecting to DDC monitoring platform and review data
- Support CNS to initiate continuous subcutaneous insulin infusion pumps for T1DM Patients
- Provide telephone/ email support to patients
- Provide Face to face and virtual Review
- Inpatient review of patients who are referred on Synergy
- Identify Diabetes complications (DKA HHS Hypoglycaemia) and educate patient
- Explore nursing interventions that assist patients with issues of compliance to treatment and identify appropriate interventions in discussion with the relevant team members.
- Work closely with other members of the diabetes team. Attend team meetings
- Establishes clear patterns for communicating and co-operating with medical and ward staff and other professionals such as general practitioners public health nurses and practice nurses
- Work closely with diabetes medical team in clinical decision-making and discharge planning for individual patients.
- Deliver structured educational opportunities to facilitate staff. This will include educational sessions for Pre-Registration and Post Registration Nurses.
- Undertake continuing professional development through formal and informal educational opportunities thus ensuring continuing credibility amongst nursing medical and allied health professionals.
