Description
We are seeking dedicated and mission-driven individuals to join our Resource Navigation Team as Resource Navigator - Community Health Workers (CHW). Community Health Workers will work from one of our Welcome Centers and support individuals and families across all areas of the county. This position focuses on addressing all aspects of the social determinants of health including food security housing stability employment healthcare access transportation and social connection. Using a trauma-informed strengths-based case-management approach you will support participants in achieving long-term stability and self-sufficiency.
Key Responsibilities
- Provide direct support to individuals and families in the community in participants homes and at various Welcome Centers as needed.
- Conduct assessments with individuals families and small groups to identify strengths needs and barriers related to the social determinants of health.
- Work collaboratively with participants to develop comprehensive action plans aimed at reducing barriers to stability.
- Offer referrals warm hand-offs and linkages to WCA programs and external community partners.
- Coordinate services across multiple community systems to ensure participants receive timely effective support.
- Maintain accurate detailed and timely documentation in required databases in accordance with agency funding and regulatory standards.
- Participate in outreach events trainings and cross-program collaboration to enhance community engagement and access to services.
Westmoreland Community Action is a non-profit organization that works to connect individuals and families to opportunities and resources throughout the community to improve their quality of life and help to reduce poverty in the community. This is accomplished through housing counseling support education intervention collaborations partnerships information referrals and networking.
Our Welcome Centers are where our staff is more easily accessible in the community. The staff at the welcome centers are navigators and are available to assist folks in getting any of our services and also to make warm hand off referrals to outside agencies when appropriate.
Requirements
- High school diploma or equivalency required; bachelors degree in a related field preferred.
- Minimum of one year of casework human services or related experience.
- Basic computer proficiency including Microsoft Office Suite (Word Excel Outlook).
- Strong verbal and written communication skills; ability to engage participants using a trauma-informed client-centered approach.
- Ability to obtain and maintain all required clearances: Act 33 Act 34 Act 114 and sex offender registry check.
- Possession of or ability to obtain Recognizing and Reporting Child Abuse certification.
- Valid PA drivers license $15000/$30000 bodily liability car insurance and a reasonably clean driving record.
- Ability to work independently and in varied environments including community-based settings.
