Were expanding and are looking for a dedicated EN to join our Access Aged Care team across the Central Coast. This is a big job and would suit a nurse who has Aged Care experience and is highly organised with strong critical thinking skills.

What Youll Do
- Work alongside RACF care teams as the on site Access Aged Care nurse.
- Facilitate GP and Specialist rounds (both in-person and telehealth) to ensure smooth timely care.
- Prepare organise and triage referrals to maximise efficiency and outcomes.
- Conduct bedside assessments and clinical reviews.
- Act as the communication bridge between RACFs GPs and families.
- Deliver excellent customer service with empathy and professionalism at every interaction
- Build strong relationships with residents families and facility partners.
- Use your clinical knowledge and IT skills to ensure quality outcomes.

What Youll Bring
- Diploma of Nursing and AHPRA registration.
- 2 years Aged Care experience (desirable)
- Strong organisational problem-solving and communication skills.
- Confidence with IT and documenting resident needs.
- A drivers licence and reliable vehicle.
