Your role as a Case Manager
As a Full-Time Case Manager specializing in Substance Use Disorder at the Hampton-Newport News Community Services Board you will play a critical role in ensuring continuity of care for individuals with mental illness intellectual developmental disabilities and substance abuse challenges. This dynamic position involves assessing individual needs developing tailored service plans and providing hands-on support in areas such as crisis intervention basic needs assistance and medication supervision. You will engage with individuals and families to deliver compassionate advocacy while coordinating services across various disciplines. The role requires creativity and adaptability to navigate resource limitations especially in crisis situations.
You will also support clients in achieving their goals and participate in discharge planning. This multifaceted role offers the opportunity to make a meaningful impact in the lives of those facing serious emotional disturbances and substance use disorders.

To excel as a Full-Time Case Manager specializing in Substance Use Disorder at the Hampton-Newport News Community Services Board candidates must possess a comprehensive understanding of community services and support systems including primary healthcare mental health and substance abuse treatment programs. Knowledge of the nature of serious mental illness and developmental disabilities is essential. Proficiency in various assessment types treatment modalities and the service planning process will enable effective development of Individual Service Plans tailored to client needs. Successful candidates will demonstrate strong skills in identifying resources and coordinating services while maintaining effective relationships with team members and external providers. The ability to engage with clients compassionately and to document their needs accurately is vital.
Independent working capabilities combined with a commitment to uphold confidentiality and comply with applicable laws will ensure the delivery of high-quality case management services to individuals facing Substance Use Disorder challenges. A Bachelors degree in Human Services and relevant experience are also required.
Knowledge Skills & Abilities
Community services and systems including primary care support services eligibility criteria and mental health developmental disability and substance use treatment programs.
Assessment types and their use in service planning.
Treatment and intervention approaches (behavior management independent living skills counseling crisis intervention discharge planning service coordination).
Service planning processes basic medication knowledge and applicable laws (HIPAA 42 CFR Part 2).
Identifying and documenting individual needs.
Using assessments and interviews to develop Individual Service Plans.
Locating and coordinating community resources and natural supports.
Coordinating services across providers.
Work effectively on a team and with community agencies.
Work independently with general supervision.
Build and maintain supportive relationships with individuals served.
Formal Training & Work Experience
- Bachelors degree in Human Services (Social Work Psychology Sociology or Counseling) with one (1) year experience providing case management services to individuals with a diagnosis of substance use disorder or co-occurring diagnosis of substance use disorder and mental illness.
- Valid Virginia Drivers License.
