We are expanding our Hospice at Home service into Hammersmith Fulham and are seeking to recruit a Nurse Manager for the Hospice to lead on this exciting development.
What would be my responsibilities as a Hospice at Home Nurse Manager
The Service is designed to deliver end of life and respite care for patients with specialist palliative care needs that wish to remain in their own home. Care is delivered in the patients home by Health Care Assistants.
The post holder will be responsible for the management of the St Johns Hospice @Home service which provides home care for patients living with in our catchment area of Westminster Kensington Chelsea South Brent and Hammersmith and Fulham.
The post holder with be responsible for the overall management of the service. You will work with two clinical support nurses and a team of Health Care Assistants. You will be supported in your role by the Community Specialist Palliative Care team lead.
Am I the right person for this role
You will have
- Significant experience of working in a Palliative Care either in the community or an inpatient care setting
- Experience of working with members of a multi-disciplinary team
- Experience of co-ordinating with external teams eg Continuing Health Care Community Nursing
- Understanding and commitment to the values our organisation
