Position Summary
The Health Systems Change Program Manager is responsible for developing delivering and refining a health systems change service to facilitate changes in workflow that support delivery of treatment referralsadvance provider training and capacity-building efforts to improve clinical practice. Key activities include leading a team developing and refining tools outlining processes strategizing and prioritizing service delivery and program enhancement. The Health System Change Program Manager demonstrates success through sound decision making stakeholder engagement and program growth.
Essential Duties
- Manages and develops an effective staff: providing effective communication leadership guidance and resources. Determines staff qualifications and competency: recruits interviews selects hires trains orients mentors evaluates coaches counsels disciplines and rewards. Establishes and monitors staff safety and regulatory compliance.
- Responsible for overall health systems change program leadership direction and guidance to ensure successful program execution in accordance with budget schedule contract and resources. Maintains current knowledge of best and promising practices for health systems change.
- Creates health systems change program business plan with short and long term goals aligned with overall department plan. Develops refines and expands strategy and methods to ensure successful program delivery.
- Proactively facilitates strategic discussion and prioritization with clients regarding program targets and deliverables. Assesses market opportunities and develops process to engage health systems.
- Develops delivers and evaluates provider-focused training initiatives including continuing education modules learning collaboratives and communities of practice. Incorporates participant feedback to improve learning outcomes and system adoption. Leads training initiatives for health care providers to strengthen evidence-based treatment delivery referral processes and e-referral adoption.
- Ensures training content reflects best practices collaborates with internal and external experts to effectively deliver training in online virtual and live settings. Ensures technical assistance meets needs of health systems to change workflow to maximize treatment delivery and referrals.
- Facilitates development of all program support materials and processes including tools presentations reports guides and/or other relevant materials. Delivers presentations and trainings to various audiences.
- Monitors and reports to clients on program deliverables. Ensures program deliverables meet client contract reporting and invoicing requirements by efficiently allocating and managing program personnel and resources for a positive program ROI.
- Works with department leaders to review evaluate develop and implement strategies for program enhancement and growth. Seeks and incorporates internal and external stakeholder feedback into services.
- Serves as subject matter expert for program in client communication and across the department. Supports department leadership to effectively onboard new program clients support contract requirements and deliverables.
- Works with Account Management team to support program sales including creating documents proposals or other materials including provider-facing training curricula marketing materials and outreach strategies to engage clinical partners..

Minimum Qualifications
