Job Responsibilities
Under general supervision the Community Health Educator will play a key role in supporting activities aligned with grant-funded adolescent health and youth development initiatives. The Community Health Worker (CHW) will play a key role in supporting activities aligned direct outreach in high-risk areas across DeKalb County engaging individuals and families where they live and gather. This includes visiting homeless encampments connecting with people who use opioids and participating in health fairs and other community events. CHW will also promote and recruit participants for the departments monthly Naloxone Response Trainings ensuring residents have access to overdose prevention education harm-reduction resources and other vital health services. The successful candidate will complete Naloxone training and become a certified Naloxone Response Facilitator enabling them to educate community members and distribute naloxone effectively during outreach activities.
A Community Health Worker is a frontline health worker who is a trusted member of and/or has demonstrated working knowledge of the community and individuals served. Under limited supervision works closely with the Community Health Worker to serve as a resource to promote maintain and improve individual family and community health. Additionally the CHW will work in partnership with all Health Assessment and Promotion (HAP) staff to provide person-centered support to individuals to help improve access to care; assist with referrals; support research through outreach and advocacy and health needs assessments advocate for individual family and community needs; and build client capacity to increase health knowledge and self-sufficiency. The role also includes coordinating outreach efforts engaging directly with community partners and representing the department at events. Evening and weekend availability is required.
Job Description and Duties:
Responsible for program activities:
- Train community partners and residents in opioid prevention strategies such as Naloxone Response Training.
- Assist with the planning implementation and evaluation of HAP outreach activities.
- Knowledge of epidemiological methods to collect organize and interpret opioid-related data to inform program strategies and reporting.
- Coordinate with the Health Promotion Specialist ensuring alignment of outreach activities with program goals.
- Assist in preparing data reports presentations and fact sheets to communicate findings to stakeholders leadership and community partners.
- Conduct and support community outreach efforts including health fairs school-based education and coalition meetings.
- Drive and operate department mobile units for outreach events and service delivery.
- Maintain accurate records of program activities outreach and evaluation metrics.
- Serve as a liaison between the health department and community organizations to strengthen partnerships and advance health equity.
Minimum Qualifications
Associate degree in business or related field from an accredited college or university and one (1) year of experience related to area of assignment. Note: An equivalent combination of knowledge education job or intern experience training or certifications that provides the necessary knowledge and skills to successfully perform the job at the level listed may be substituted year-over year.
PREFERRED QUALIFICATIONS:
- Masters degree in public health Epidemiology or related field.
- Experience coordinating staff volunteers or community health workers.
- Training or experience in harm reduction or opioid overdose prevention.
- Bilingual proficiency to support diverse populations.
- Familiarity with field-based service delivery.
- Experience with community outreach and engagement in diverse communities.
- Knowledge ofsubstance use disorders (SUD) homelessness and social determinants of drug use.
- Strong communication and presentation skills both oral and written.
- Valid drivers license and clean driving record.
- Proficiency with Microsoft Office Suite and data analysis tools.
- Willingness and ability to work evenings and weekends as required.
- Ability to work independently and collaboratively with a multidisciplinary team.
