Classification - IN31/IN32 depending on experience
Salary - $28.8473 to $31.65 per hour 12% Superannuation

About the role
Social Support Groups are held in Coleraine Campus 2 days per week servicing the Coleraine and Merino Residents.
The Western District Health Service Social Support Groups provide day programs to support people living in our community catering for people living in their own homes within the Hamilton Penshurst Coleraine and Merino area over 5 days per week. We strive to ensure our members live well achieve and feel valued build on their skills and interests stay connected within the community and maintain independence.
The Social Support Group department has a great opportunity for an enthusiastic and dynamic Social Support Group Assistant to join our team in Coleraine.
As part of this role you will be required to work in a team of 2 staff co leading driving the bus for pick ups and drops offs from and to members homes and on outings facilitate a creative dynamic productive program in a safe working environment promoting innovative and effective programs to support members of the community to live well and feel valued.
You will build partnerships to maximise independence and wellness by supporting the members to build on their strengths and interests to achieve identified goals and monitor members health and wellbeing.
In addition develop and maintain teamwork communicate effectively with all staff in SSG department volunteers members and carers. Work as a co leader in program planning delivery evaluation and building on community connections.
Assist the Manager and/or Coordinator (based in Hamilton) with administrative duties as required.

The successful candidate must have or obtain a National Criminal Police Check. Depending on the role a Working with Children Check Clearance Disability Worker Exclusion Scheme check and/or NDIS Worker Screening Check may also be required.
