Role Purpose
The Adult Social Care Hospital Social Worker is a key member of the Integrated Discharge Team responsible for assessing patients social care needsparticularly older adults and those with complex requirementsto facilitate timely and safe hospital discharges. The role aims to prevent unnecessary admissions and readmissions by ensuring patients receive appropriate community support.
Key Responsibilities
Conduct comprehensive social care assessments for patients within tight timeframes.
Identify and evaluate complex needs including safeguarding concerns mental capacity issues and fluctuating conditions.
Develop and implement person-centred care plans in collaboration with patients families carers and multidisciplinary teams.
Advise on and facilitate access to appropriate social care services equipment and home adaptations.
Liaise effectively with internal and external partners including health professionals voluntary organisations housing services and other local authority departments.
Participate in multidisciplinary team meetings and case conferences to support integrated care planning.
Identify and respond to adult safeguarding concerns in line with local policies and procedures.
Work collaboratively with other agencies to protect vulnerable adults.
Coordinate hospital discharge plans including arranging care packages residential care or community-based support.
Ensure all arrangements are in place for safe patient transitions from hospital to home or suitable environments.
Maintain accurate timely and professional records including assessments care plans and safeguarding documentation.
Requirements
Qualifications: Recognised social work qualification (e.g. Degree in Social Work DipSW CQSW).
Registration: Current registration with Social Work England.
Experience: Significant experience in adult social care ideally within a hospital or fast-paced environment.
Knowledge: Strong understanding of relevant legislation and frameworks including the Care Act 2014 and Mental Capacity Act 2005.
Skills: Excellent assessment communication negotiation and problem-solving abilities. Able to work independently and collaboratively within a team.
