About the Role
We are seeking compassionate dedicated nurses to provide high-quality in-home care to patients. As a Home Health Nurse youll deliver individualized care support patient independence and build meaningful relationships with those you serve.
Responsibilities
Conduct in-home assessments and develop patient-centered care plans
Provide skilled nursing care (medication management wound care monitoring chronic conditions etc.)
Educate patients and families on disease management and safety
Document care accurately and timely according to agency standards
Collaborate with an interdisciplinary team to coordinate care
Qualifications
Active RN or LPN license in Kansas
Strong communication and clinical assessment skills
Compassion professionalism and a commitment to quality care
