A great opportunityhas arisen for a motivated professional to join the Community Support Service. This service provides support andassistancefor people with mental health/AOD issues living in the community.
About thisrole
To provide support andassistancefor people with mental illness living in the community encouraging them to build on their strengths and the strengths in their community encouraging their needs for developing skills accessing community resources developing functional relationships and independence.
What youllbe doing
- Working with a defined caseload of individuals to enhance their community living skills and to develop self-management of their lives
- Assisting clients to make connections with appropriate services/agencies and ensure required services are received
- Active participation in shared planning with the client theirwhānau/family other clinical and non-clinical providers
- Communication and liaison with health and community agencies
- Maintainingaccuratehealth records and statistical data.

It is desirable that you have a New Zealand Certificate in Health and Wellbeing Level 4 - Mental Health and Addiction Support (or equivalent qualification) or are committed to studying towards one.
Experience in the mental health and disability sector would be an advantage.
Applicants must hold a current full clean drivers license.
You will have a cultural awareness knowledge of and demonstrable commitment to the principles of TeTiritio Waitangi.
Eligibility
Applicants for this position must have the legal right to work in NZ and consent to undergo acriminal record checkprocess.
