About the role
The role of the Senior Treatment Coordinator is to provide individual substance abuse counseling and case management to clients of Casa Esperanza Inc.s residential program using a motivational counseling approach and providing individual assessment treatment planning and case management. The Senior Treatment Coordinator acts as the assistant program director and helps coordinate all admissions treatment reporting ESM data and client fee collection. Bilingual (Spanish and English) Required. As part of the Casa Esperanza Inc. team the Senior Treatment Coordinator will provide services in a fully integrated holistic and collaborative manner using a team-treatment approach to ensure the coordination of all client services both across Casa programs and with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessary follow-up and assignments; and ensure wrap-around support services are in place to maximize treatment effectiveness and clinical outcomes.
What youll do
- Serves as HIV/AIDS Coordinator
- Works closely with Program Director and Clinical Supervisor to coordinate intakes admissions and
- Discharges to and from the program
- Conducts psychosocial assessments for clients in the program and makes referrals to additional mental health and developmental support services when needed
- Maintains working knowledge of Residential Program Casa Esperanza menu of services ASAM criteria and intake process
- Provides the 12 core counseling functions to each assigned client: screening intake orientation assessment treatment planning counseling case management referral education crisis intervention record-keeping
- Facilitate access to and coordination of needed services HIV clients including medical and dental care prescription services medical treatment compliance enrollment in Massachusetts HIV Drug
- Assistance Program and mental health/substance abuse counseling and social services.
- Manages HIV/AIDS data collection and reporting systems
- Conducts initial assessment for each client and generates individualized treatment and service plans based on Casa Esperanza Inc.s Clinical Logic Model
- Develop aftercare plan with the client
- Coordinates internal and external services i.e. legal medical and housing employment and training etc.
- Provide ongoing education regarding the fundamentals of HIV/AIDS HIV medications and strategies for personal risk reduction.
- Provides the 12 core counseling functions to each assigned client: screening intake orientation assessment treatment planning counseling case management referral education crisis intervention record-keeping
- Works closely with clinical team to ensure client-individualized treatment and service plans are completed
- Conducts program groups as necessary (relapse prevention self-esteem orientation group solutions-focused group)
- Facilitates group interventions as required (including Trauma Parenting Education etc.)
- Documents all client encounters and progress completes necessary reports and maintains up-to-date client files
- Works closely with the Residential Operations Manager in collecting and processing client EBT and client fees maintaining appropriate documentation upon receipt
- Participates in clinical team meetings DCF and family team meetings to address client needs
- Manages the monthly and weekly review of medications
- Manages crisis interventions as needed
- Conducts urine screening for clients as needed
- On Call (rotating)
- Meets weekly with the Clinical Program Director for supervision
- Other duties as required
Qualifications
- Skills necessary for the position:
- Ability to use Motivational Counseling/Motivational Interviewing techniques
- Familiarity with Cognitive Behavioral Therapy
- Demonstrated capacity to make sound clinical decisions and manage crises
- Valid Massachusetts Drivers License
- Computer skills minimum Microsoft Office
- Experience with homelessness co-occurring disorders and people living with HIV/AIDS
- Knowledge of substance abuse field including the AA/NA 12-step philosophy
- Knowledge of appropriate community resources
- Knowledge of residential treatment models
- Strong verbal and written communication skills
- Crisis management skills and group intervention skills with children and families
- Ability to work as a member of a team and independently
- Must be knowledgeable of HIPAA regulations and how to assess the need of mental health hospitalization
- Ability to meet the physical requirements of the position (see attached)
- Ability to meet the training requirements of the position (see attached)
- Education/Training required:
- Masters degree in Social Work preferred
- Bachelors degree in Psychology counseling or equivalent
- All candidates should be willing to become CADAC certified (or a reasonable equivalent).
- Experience working with Domestic Violence
- Experience providing survivor-centered trauma-informed care
- Experience delivering strengths-based treatment
- Bilingual Spanish/English required
- CPR certified
