Job Summary

The Care Partner supports Aboriginal Elders and people living with disability to stay safe independent connected and culturally supported at home and in the community. The role provides non-clinical independence-focused support including domestic assistance social support basic personal care meal preparation community participation reablement activities and safety monitoring with early identification of risks. Care Partners follow individualised support plans and escalate clinical or safety concerns to the Care Partner Lead and Clinical Nurse Lead.

GEGAC is an Aboriginal Community Controlled Organisation based in Bairnsdale Victoria. Consisting of about 190 staff GEGAC is a Not-for-Profit organisation that delivers holistic services in the areas of Primary Health Social Services Elders & Disability and Early Childhood Education.

GEGAC advocates for the self-determination of the Aboriginal community by prioritising services that enhance social emotional and spiritual wellbeing with culture at the heart of everything we do.

Position Purpose

The Care Partner supports Aboriginal Elders and people living with disability to remain safe independent connected and culturally supported in their homes and community.

The role delivers non-clinical independence-focused support including:

Domestic assistance

Social support (group and individual)

Basic personal care (within SCHADS Level 3 scope)

Meal preparation

Community participation

Reablement-focused activities

Safety and wellbeing monitoring

Early identification of risks and concerns

Cultural connection support

Care Partners follow individualised support plans and escalate any clinical or safety concerns to the Care Partner Lead and Clinical Nurse Lead.

Key Selection Criteria

  • Demonstrated experience supporting older people to live independently at home.
  • Understanding of the Strengthened Aged Care Quality Standards and rights-based care.
  • Experience delivering reablement wellbeing and independence-focused practice.
  • Ability to support older people with diverse and more complex needs.
  • Strong coordination navigation and problem-solving skills.
  • Ability to guide and mentor staff and build team capability.
  • Strong documentation planning and organisational skills.
  • Demonstrated understanding of safety early risk identification and safeguarding.
  • Excellent communication relationship-building and interpersonal skills.

Qualifications and Registrations Requirement (Essential or Desirable).

  • Certificate III or in Community Services Aged Care Individual Support Disability Social Services or related field. ESSENTIAL
  • Additional training in reablement goal-based support Support at Home or service coordination. HIGHLY DESIRABLE
  • Ability and willingness to maintain current First Aid and CPR certification to ensure safe response to incidents medical changes falls or emergencies in client homes. ESSENTIAL
  • Valid Drivers Licence: A current and unrestricted Victorian Drivers Licence is essential as the role involves regular travel across the Gippsland and East Gippsland region to engage with community staff service sites and external stakeholders. ESSENTIAL
  • A satisfactory Victorian Working with Childrens Check and a satisfactory National Police Check (Aged Care / Vulnerable Persons Sector) and NDIS Worker Screening Clearance ESSENTIAL
  • Ability to meet occupational vaccination requirements for workers supporting vulnerable people including providing evidence of immunisation history and recommended vaccinations and proof of COVID-19 vaccination to align with GEGAC requirements ESSENTIAL
  • A person of Aboriginal and or Torres Strait Islander background DESIRABLE
