The Emory Valley Center
Emory Valley Center is a nonprofit organization located in Oak Ridge Tennessee dedicated to serving individuals with developmental disabilities since 1955. We provide a variety of services across 16 East Tennessee counties including supported living day programs employment assistance early intervention family support and therapies such as speech occupational and behavioral health. Our commitment to inclusivity extends to our Emory Valley Early Learning Center a preschool licensed by the Department of Human Services. Our new facility opened in 2016 enhances our ability to provide meaningful services aimed at promoting independence dignity and community inclusion for individuals with disabilities.
Responsibilities:
The Case Manager acts as the key contact for the individuals we support collaborating closely with EVC staff and external professionals to create implement and sustain customized support plans. With a strong sense of independent judgment you will oversee assessments treatment planning and service delivery ensuring they align with Person-Centered practices while meeting our organizations goals and DDA/MCO initiatives. Additionally this role encompasses advocacy and liaison funcions for individuals and their families promoting effective communication among diverse stakeholders to enhance service delivery and address any challenges that may arise in different areas of their lives.
- Act as the primary liaison for families and professionals championing support plans and driving person-centered planning through the development of tailored PCSPs. Establish meaningful connections with families and conservators through monthly check-ins while collaborating with the Circle of Support and care teams to share progress and address
- Act as the main liaison for families and professionals driving person-centered planning through tailored PCSPs. Build connections with families via monthly check-ins and collaborate with support teams to address individual needs.
- Manage medical emergencies hospital care discharge planning and follow-ups. Oversee scheduling analyze health records process paperwork and coordinate prescriptions to ensure compliance with policies.
- Support Medicaid redeterminations appeals and develop PCSPs for private pay individuals. Maintain and update ITSPs/PCSPs review medication logs and refine plans for accuracy and compliance.
- Submit timely Provider Reviews conduct monthly home visits adapt PCSP drafts and complete annual paperwork including consents and Human Rights compliance. Monitor individual rights manage therapy orders and handle insurance approvals.
- Encourage technology use for independence set community and work goals and partner with Clinical Services for mental health support. Provide after-hours assistance as needed.
- Balance office and fieldwork track time accurately and take on additional duties as assigned ensuring excellence in all responsibilities.
Requirements- A Bachelors degree in a relevant field or a TN Licensed Practical Nurse (LPN) designation is preferred.
- Medical Assistant certificates can be considered as an alternative to LPN provided you have at least one (1) year of relevant experience.
- If you have five (5) years of experience in healthcare or DDA this can also be an acceptable substitute for LPN or Medical Assistant certification.
- Previous experience in case management is highly desired.
- Successful completion of DDA and Managed Care Organization (MCO) online training along with First Aid CPR and Medication Certification courses is required within the first 90 days of employment in addition to any other mandatory training.
