Alfred Health is a leader in health care delivery improvement research and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne from ambulatory to inpatient and home and community-based services. On 1 January 2026 Alfred Health will come together with Kooweerup Regional Health Service Peninsula Health Bass Coast Health and Gippsland Southern Health Service to create Bayside Health a newly merged health service with more than 22000 employees caring for a community of over 1.2 million Victorians. The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce providing new training education and career opportunities. Subject to final approval on 1 January 2026 all employees from the five health services will transfer to Bayside Health.
About us
Alfred Health Renal Service provides comprehensive renal care across inpatient outpatient and community settings including dialysis and transplantation. Services span acute and chronic kidney disease and are delivered through multiple sites including Caulfield Sandringham and partner locations like Peninsula Health. Dialysis options include in-centre home and satellite units and transplant services cover deceased live donor and multi-organ transplants.
About the role
The Renal Patient Navigator is a vital member of the multidisciplinary renal care team and serves as the primary point of contact for patients carers and community referrers. The role supports smooth and timely patient flow through the renal service by coordinating access to care and managing all related inquiries including appointment scheduling changes and clarifications. When clinical input is required the Navigator ensures patients are promptly connected with the appropriate healthcare professionals.
Working closely with patients carers and clinical teams the Navigator coordinates care pathways to ensure patients receive the right care at the right time and place. This includes managing diagnostics treatment and review appointments optimising scheduling to reduce patient burden by coordinating multiple appointments or offering telehealth options when suitable.
The role also provides essential administrative support to the renal team including the registration and processing of nursing encounters off-site clinics and multidisciplinary team meetings (MDMs). The Navigator ensures accurate preparation and documentation of patient information to support efficient and effective care delivery.
