Where youll be working:
Manning Base Hospital has a great opportunity for a Social Worker to provide high quality clinical services and leadership as part of the multidisciplinary team for patients and their families/carers in the Lower Mid North Coast Sector of the Hunter New England Local Health District. The position is responsible for the overall Social Work service provision to the Renal services and other areas of the hospital as required.

About the role:
In this role you will partner with patients and their families to enhance overall wellbeing and health through effective disease management supporting individuals to maintain quality of life while adapting to chronic kidney disease enabling functional rehabilitation and continuation of personal life goals and providing adjustment counselling to families and carers.

The successful applicant will receive ongoing support with a comprehensive orientation and training provided by multiple members of the team along with continued access to education and professional development opportunities.

About you:
The successful applicant will bring sound clinical knowledge of renal disease and its psychosocial impacts with the ability to assess patient needs develop individualised care plans and provide counselling and emotional support to patients and their families. They will be confident managing complex and emotionally challenging situations advocating for patients and navigating community financial and government support services while maintaining ethical practice accurate documentation and confidentiality.

You will work effectively as part of a multidisciplinary team and demonstrate strong communication problem-solving and adaptability in a dynamic clinical environment. A compassionate and resilient approach is essential along with the ability to work sensitively with diverse communities including Aboriginal and Torres Strait Islander peoples.

Requirements:

  • Bachelor in Social Work or equivalent with eligibility for membership with Australian Association of Social Work (AASW).
  • Current unrestricted drivers licence and the ability and willingness to travel for work purposes which may involve driving long distances.
