We are seeking a compassionate dedicated and highly skilled Registered Nurse (RN) to join our home care team as a Home Care Field Supervisor. As a Field Supervisor you will provide clinical oversight support caregivers in the field and ensure all services meet state regulations and agency standards.

Key Responsibilities

• Conduct in-home supervisory visits with clients to assess care quality safety and client satisfaction.

• Perform initial assessments re-assessments and ongoing evaluations of client conditions and needs.

• Develop update and implement individualized care plans according to state requirements and agency guidelines.

• Train mentor and support field staffincluding HHAs CNAs and other caregiversto ensure competency and compliance.

• Provide clinical guidance and act as a resource for caregivers regarding client needs care techniques and best practices.

• Monitor documentation for accuracy completeness and compliance with state regulations.

• Maintain timely communication with office staff clients family members and care teams.

• Participate in on-call rotation as needed to support the agency and ensure continuity of care.

• Identify concerns in the field and take appropriate corrective actions to maintain high standards of service.

• Ensure all care is delivered in accordance with company policies licensing standards and ethical practices.

Qualifications

• Must hold an active unrestricted Registered Nurse (RN) license.

• Minimum 1 year of nursing experience required (home care experience preferred).

• Strong understanding of home care regulations care plans and assessment procedures.

• Excellent communication leadership and interpersonal skills.

• Ability to work independently manage time effectively and handle multiple client needs.

• Compassionate patient-centered mindset with a commitment to high-quality care.

• Valid drivers license and reliable transportation for field visits.
