Your next position could be providing evidence-based and recovery-oriented support to people at risk of homelessness discharged from acute mental health wards and/or supported by community mental health services. You will assist clients to achieve their identified goals and aspirations provide support with day to day tasks and activities and develop professional working relationships with stakeholders to achieve best possible outcomes for clients. Working as part of a wider team you will also support the recovery support workers to achieve positive outcomes with clients on their caseload ensure they are meeting the program requirements and will be provided with on the job supervision on an ongoing basis.
Your Key Responsibilities:
- Foster a positive recovery-focused culture that values lived experience genuine relationships mutual respect empowerment and personal growth.
- Deliver strengths-based outcomes-focused support in line with RFQs Model of Practice/Peer Work Framework including skills development capacity building and community connection.
- Conduct safety assessments recovery plans and reviews to ensure safe high-quality and client-centred service delivery.
- Work collaboratively with internal and external stakeholders to help clients achieve their goals and access appropriate supports services and opportunities.
- Maintain accurate timely documentation and reports supporting contractual obligations KPI monitoring and service quality.
- Participate in networks and community meetings to stay informed about sector developments and enhance opportunities for clients and RFQ.
- Contribute to continuous improvement by identifying opportunities sharing innovative ideas and supporting effective team dynamics.
You will need:
- Degree in a human services related field and demonstrated previous relevant experience working with people with mental illness; or
- Certificate IV/Diploma in Mental Health or equivalent and significant demonstrated previous experience working with people with mental illness.
- A commitment to RFQs mission and values Professionalism competence and high ethical standards
- Authenticity and a genuine client-focused approach supported by an approachable caring and clear communication style.
- Ability to deliver quality client support while developing and maintaining strong professional relationships with external stakeholders.
