Are you passionate about making a difference in the lives of youth and families navigating complex mental health needs Northwestern Counseling & Support Services Inc. is seeking a dedicated Acute Care Consultant to join our dynamic Stabilization Support Team. This role offers the opportunity to lead with compassion coordinate care across systems and create meaningful impact in the lives of our most clinically acute clients.
As an Acute Care Consultant youll work in close partnership with the Childrens Home and Community Based Program Manager to support team operations guide clinical processes and ensure timely access to higher levels of care for youth in the Franklin Grand Isle region. Youll serve as a key liaison between NCSS and community partners such as DCF DMH and local providershelping to break down barriers and build bridges that lead to lasting stability for the children and families we serve.
The ideal candidate will bring good collaboration experience strong organizational skills and a family-centered approach to care coordination. Youll offer consultation and support to team members contribute to quality improvement efforts and occasionally provide direct services for clients receiving respite psychiatric care or waiting for placement.
This position offers a meaningful blend of systems-level coordination team leadership and direct clinical impact all within a supportive and mission-driven environment.
Key Responsibilities:
- Assist in managing the daily operations of the Stabilization Support Team
- Coordinate access to higher levels of care for FGI-designated youth
- Collaborate with DCF DMH and other partners to ensure appropriate services are in place
- Provide guidance consultation and training to HCBS Childrens team members
- Support and track Coordinated Services Plans waiver documentation and compliance
- Contribute to staff onboarding morale and team development
- Maintain accurate timely documentation and uphold Medicaid standards
Position requires:
- Flexibility in scheduling and travel throughout region
- Strong communication skills and a collaborative spirit
- Commitment to professionalism and quality care

Hiring rate is based on experience education and internal equity. Final compensation will be determined in accordance with NCSS policy and applicable laws.
Qualifications
Bachelors degree in Human Services Social Work Psychology or a closely related field.
