JOB TITLE: Licensed Clinical Social Worker

REPORTS TO: Director of Behavioral Health

The following statements reflect the general duties responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.

RESPONSIBILITIES:

  • Engages patient(s) in therapeutic process through one-on-one therapy and/or group therapy sessions
  • Provides therapeutic intervention to patients with behavioral health needs including individual family and group psychotherapy community work crisis intervention case management other treatment modalities
  • Prepares psychosocial histories/reports and maintains treatment records in accordance with clinic and regulatory guidelines
  • Conducts psychosocial assessments and develops and implements treatment plans
  • Obtains collateral information from patients family members outside treatment providers schools and other relevant team in the assessment of medical and behavioral health needs
  • Responds to referrals and consultation requests from primary care providers
  • Participates in agency Quality Assurance activities and case discussion meetings
  • Provides task supervision and consultation to social work interns other clinical trainees and volunteers if needed
  • Other duties as assigned.
QUALIFICATIONS:
  • Masters Degree in Social Work from an accredited school of social work
  • New York State License required
  • Strong verbal and written communication skills
  • Previous work experience in behavioral health or health setting preferred
  • Ability to travel to other sites as needed
  • Bilingual in English and Spanish/Creole required.
