As a member of the Palliative Care team the Social Worker works to assure continuity and coordination of care for outpatient adult oncology patients/families. Services include but are not limited to; assessing patient/family strengths in regards to social environmental psychological financial ethnic spiritual and cultural influences related to living with cancer providing one-to-one acute emotional support to patients and families enhancing client access to resources facilitating group and family conferences participating in patient care planning rounds and supporting continuity of care.
Qualifications
Masters in Social Work degree from an accredited school required.
Licensed Social Work Associate-Advanced (LSWAA) or Licensed Social Work Associate Independent Clinical (LSWAIC) required. Licensed Independent Clinical Social Worker (LICSW) or Licensed Advanced Social Worker (LASW) with the Washington State Department of Health under Chapter 18.225 RCW preferred.
Two years experience as a social worker in a healthcare setting preferred. Experience working with patients who have a chronic or acute illness preferably in an oncology setting.
