Job Summary
DESCRIPTION
Compensation: Salary annually depending on experience plus performance-based incentives.
We are seeking an experienced and compassionate RN Clinical Manager with a strong background in home health to join our leadership team. This individual will play a pivotal role in overseeing clinical operations ensuring the highest quality of patient care and maintaining regulatory compliance within our home health agency. The RN Clinical Manager will serve as a mentor and leader to the clinical staff driving performance patient outcomes and operational excellence.
The Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel:
- Develops plans implements analyzes and organizes clinical operations.
- Supervise and support clinical staff including RNs LPNs therapists aides and other home health professionals.
- Review and approve patient assessments care plans and documentation.
- Conduct routine case conferences and participate in interdisciplinary team meetings.
- Monitor clinical performance caseloads and patient outcomes.
- Provide field supervision and performance evaluations as needed.
- Collaborate with the intake scheduling and administrative teams to ensure efficient operations.
- Assist in hiring onboarding and training new clinical staff.
- Participate in quality improvement initiatives and ensure documentation accuracy.
- Serve as a liaison between clinical staff physicians patients and families.
Required Experience/Skills
- Graduate of an accredited School of Nursing.
- Registered Nurse (RN) license in good standing required.
- Clinical leadership experience in a Medicare-certified home health agency.
- Strong understanding of PDGM QAPI and care coordination.
- Passion for delivering high-quality patient-centered care in a home setting.
- Must know OASIS and two years as a Registered Nurse with at least one year of management experience in home care.
