In this dynamic role youll provide hands-on case management and employment-focused support guiding clients as they explore career options secure competitive jobs and navigate their path toward long-term success. Under the supervision of a Licensed or Qualified Mental Health Professional youll play a key role in diagnostic assessments treatment planning and monitoring progress throughout the recovery journey. Youll also collaborate closely with a multidisciplinary ACT (Assertive Community Treatment) team ensuring seamless coordination of care to strengthen treatment plans enhance safety and help clients achieve their goals.

Responsibilities

  • Performs diagnostic assessments and collects diagnostic information to determine an individualized treatment plan for the client.
  • Plans treatment modalities coordinates treatment plans and follows clients through the treatment process.
  • Provides counseling problem-solving and community support (individual and group).
  • Requests consultation for clients that includes but is not limited to providers social workers and nursing staff when necessary.
  • Evaluate the clients treatment plan through follow-up assessment and makes adjustments based on clinical findings.

Qualifications

Education:

  • Requires bachelors degree with at least 30 semester hours or equivalent quarter hours in a human services field including but not limited to sociology social work counseling psychology or human services.

License(s)/Certification(s):

  • Valid drivers license.
  • Crisis Prevention Institute (CPI) training within 6 months of hire.

Experience:

  • Experience working with persons with severe and persistent mental illness.
