As part of the Ambulatory Clinical Support Team this role leads the integration of SQCN/SACO Pharmacy services into value-based care models focusing on medication optimization and chronic disease management across populations. The supervisor oversees clinical program design and daily operations provides support for staffing by participating in daily workflow activities and performance tracking to align pharmacy outcomes with payer and organizational goals. A key component of this role is workforce leadership: the supervisor actively supports staffing needs by participating in daily workflow monitoring performance metrics and fostering a high-performing adaptable team.
Leads and coordinates the development and maintenance of clinical and operationalpharmacy processes and programs
Develops implements and evaluates pharmacy services protocols policies & procedures
Models practice consistent with organizational goals customer service clinical effectiveness and standards of care
Successfully leads local and system implementation of various system initiatives
Supports the training of new employees and participates in the creation and maintenance of training materials
Education
Bachelors Level Degree in Pharmacy (Required)
Doctorate Level Degree in Pharmacy (Preferred)
Certification/Licensure
Virginia Pharmacist License (Required)
Experience
Minimum 3 years of Clinical or Ambulatory Pharmacy Experience (Required)
Residency Trained (Preferred)
1 year of Leadership Experience (Preferred)
Pharmacy Board Certification (Preferred)
